C-YES (Child and Youth Evaluation Services) In New York

C-YES is a DOH department that will apply for HCBS directly without going through a Health Home. This waiver process is generally used to help children who are on private insurance receive a Medicaid waiver in order to receive HCBS.

Additionally, children can apply this way if they are not interested in Health Home Care Management Services and only want HCBS.

Children who are using another version of a Medicaid care coordination (such as having an early intervention coordinator so they cannot also be in a Health Home) can also receive HCBS through C-YES.
